Want to design your own laser appointment cards? These multi-part perforated cards, which are designed for use with OPMS or Orthotrac software, can be customized to feature the design and colors of your choice.

Each 8-1/2 x 5-1/2 perforated card has 4 sections; the bottom right section is a reminder sticker for patients to place on their calendars. Printed on 24# bond paper in your choice of one of our 12 standard ink colors (or pay just a little extra to specify any PMS color).

Orthodontic “do’s and don’ts” (i.e. standard advice for patients with braces) can be pre-printed on the back side in black ink, if color desired on back side, please specify. Your logo can be added, will need to send Vector file for logo.

8-1/2 x 5-1/2; vertical perforation 6-1/2” from left; horizontal perforation 2-3/4” from top; 24# bond paper.  For a nominal fee enhance your image by choosing a PMS color to coordinate with your website or office decor.
