The top left section will list the patient’s next few appointments – making it easier to schedule patients way in advance. The bottom left section provides a work/school excuse. The top right shows financial information, such as balance due or past due amounts. And the bottom right is a sticker showing the next appointment, for the patient to place on their calendar at home.

Orthodontic “do’s and don’ts” (i.e. standard advice for patients with braces) are pre-printed on the back side.
